Nursing Intervention Strategy:
- Pre-Infusion Assessment & Preparation: Review recent lab values (BUN, creatinine, electrolytes, magnesium, CBC). Obtain IV access. Begin the pre-hydration protocol as ordered, typically 1-2 liters of normal saline over 2-4 hours before the cisplatin infusion.
- During Infusion: Administer cisplatin as per protocol, usually over 1-6 hours depending on dose. Continue concurrent hydration with saline. Monitor for acute reactions.
- Post-Infusion Management: Continue aggressive hydration with saline for several hours after the infusion is complete. The goal is to maintain a urine output of 100-150 mL/hr.
- Ongoing Monitoring & Education: Teach the patient to report any hearing changes, ringing in the ears, numbness/tingling in hands/feet, or decreased urine output. Schedule follow-up labs to monitor renal function and electrolytes.
Patient Safety and Precautions: Never administer cisplatin if the patient is significantly dehydrated or has impaired renal function (elevated creatinine) without consulting the oncologist. Mannitol or furosemide may sometimes be co-administered to force diuresis, but hydration remains key.

Critical Calculation: Accurately calculate the infusion rate for hydration fluids. For example, 1 L (1000 mL) over 2 hours = 1000 mL / 120 min = ~8.3 mL/min. Using a pump, set to 500 mL/hr.
