Clinical Scenario: You are the triage nurse in a busy ED. Mr. Johnson, a 72-year-old with a history of COPD, is brought in by family. He is leaning forward in his chair (tripod position), his nostrils are flaring, and you can see the sternocleidomastoid muscles in his neck contracting with each breath. His lips have a slight bluish tint. His spouse says he's been increasingly short of breath for the past 6 hours and his home albuterol inhaler isn't helping.
Nursing Intervention Strategy:
- Immediate Assessment (ABCs):
- Airway: Listen for sounds (stridor, gurgling). Ask "Can you speak?" He nods yes but in short phrases.
- Breathing: Rate, rhythm, depth. Count respirations (likely >24/min). Auscultate lung sounds (expect diminished breath sounds, wheezes, or crackles). Apply pulse oximeter (reads 88%). Observe for cyanosis and use of accessory muscles.
- Circulation: Quickly check radial pulse (rate, rhythm), skin color/temperature, and capillary refill.
- Immediate Actions:
- Call for help and alert the physician/advanced practice provider immediately.
- Initiate supplemental oxygen as per protocol or provider order. Watch out for confusion! For some COPD patients with chronic hypercapnia, high-flow oxygen can depress the respiratory drive. Start with low flow (e.g., 2 L/min via nasal cannula) and titrate to achieve SpO2 of 88-92% (target per COPD guidelines).
- Prepare for arterial blood gas (ABG) draw to assess pH, PaO2, and PaCO2.
- Assist the patient into a position of comfort that maximizes lung expansion (high-Fowler's, leaning over bedside table).
- Ongoing Monitoring & Care:
- Continuous SpO2 and cardiac monitoring.
- Administer prescribed nebulized bronchodilators (e.g., albuterol/ipratropium) and corticosteroids.
- Provide calm, clear explanations to reduce anxiety, which can worsen dyspnea.
- Monitor for changes in mental status (lethargy, confusion), which can indicate worsening hypercapnia.
Patient Safety and Precautions:
- Oxygen Caution: Remember the risk of oxygen-induced hypoventilation in COPD patients. Monitor respiratory rate and effort closely after initiating O2.
- Fall Risk: A dizzy, hypoxic patient is a high fall risk. Keep the bed in low position, call light within reach.
- Infection Control: COPD exacerbations are often triggered by infections. Practice good hand hygiene and use appropriate PPE.
