학습 자료 둘러보기 →Severe allergic contact dermatitis from poison ivy is a type IV hypersensitivity reaction driven by T-cells, not histamine. For widespread or severe cases, oral corticosteroids like prednisone are first-line to suppress the systemic inflammatory cascade.
A typical adult regimen is 0.5 to 1 mg/kg/day of prednisone, tapered over 2 to 3 weeks to prevent rebound dermatitis. Short, 6-day dose packs are often insufficient and associated with relapse.
Topical antihistamines are ineffective and may cause sensitization. Do not confuse the mechanism with type I reactions where antihistamines are appropriate. Ensure no contraindications to systemic steroids exist.
