A heart rate below 100 bpm mandates immediate initiation of positive pressure ventilation (PPV) with 21% oxygen for term infants, titrating oxygen concentration to achieve target oxygen saturations. Central cyanosis involving the trunk and mucous membranes always requires intervention, whereas acrocyanosis limited to extremities is a benign transitional finding.
Expect irregular respirations with brief apneic pauses under 20 seconds (periodic breathing) as the respiratory control center matures. Vernix caseosa should remain intact on the skin as it provides natural antimicrobial protection and moisture retention. Acrocyanosis of hands and feet typically resolves within 24 to 48 hours as peripheral circulation stabilizes. A strong cry indicates adequate lung expansion and effective clearance of fetal lung fluid.
Initiate the neonatal resuscitation algorithm when any of the following are present: heart rate below 100 bpm despite adequate stimulation, gasping or absent respirations, persistent central cyanosis, or poor muscle tone. Effective ventilation remains the single most critical intervention, as neonatal bradycardia is almost universally caused by respiratory failure rather than primary cardiac pathology. Reassess heart rate every 30 seconds during resuscitation to guide escalation or de-escalation of interventions.
