Lochia may also be increased in amount due to impaired uterine involution and the inflammatory process.
Differential Assessment for Postpartum Fever: When a postpartum client presents with fever, a systematic assessment is required to rule out other common sources. Mastitis presents with unilateral breast tenderness, erythema, and warmth. Deep vein thrombosis (DVT) presents with unilateral leg pain, swelling, and a positive Homan's sign. Wound infections at an episiotomy or cesarean section site present with localized erythema, edema, and possible purulent drainage or wound separation.
Nursing Priority: Immediate reporting of foul-smelling lochia with fever is critical. Early recognition and antibiotic therapy prevent progression to severe sepsis, a leading cause of maternal mortality. Vital signs, fundal assessment (often boggy and tender), and lochia characteristics (color, odor, amount) are the priority nursing assessments.
Management Considerations: Treatment typically involves broad-spectrum intravenous antibiotics. Supportive care includes hydration, pain management, and monitoring for complications such as peritonitis or pelvic abscess. Client education should focus on perineal hygiene, recognizing signs of infection, and completing the full course of prescribed antibiotics.
