Procedure: Apply a chemical or ice pack wrapped in a soft cloth or disposable cover to the perineum for 15-20 minutes every 2-4 hours. Never apply ice directly to the skin to prevent tissue injury.
Assessment: Before and after application, assess the perineal area using the REEDA scale (Redness, Edema, Ecchymosis, Discharge, Approximation of wound edges) to monitor healing progress.
Patient Education: Instruct the patient to report any signs of excessive numbness, skin discoloration, or increased pain. Advise that cold therapy is most effective in the first 24-48 hours; after this period, the provider may transition to moist heat to promote circulation and healing.
Contraindications: Avoid cold therapy in patients with known cold allergies, Raynaud's phenomenon, or compromised circulation in the perineal area.
