1. Initial Verification (Always First)Before touching the vial, perform a triple-check of the medication label against the Medication Administration Record (MAR). Confirm the medication name, dosage strength, and expiration date. An expired or incorrect drug must never be used, as sterility and chemical stability cannot be guaranteed. This step aligns with the first "Right" of medication administration: the right drug.
2. Gather and Calculate SuppliesSelect the correct diluent type and volume as specified by the manufacturer's package insert. Calculate the final concentration needed to draw up the prescribed dose. Do not substitute diluents or alter volumes, as this can change the medication's osmolarity, stability, or absorption rate.
3. Aseptic Technique and Diluent AdditionCleanse the vial's rubber stopper with an alcohol swab and allow it to dry. Draw up the precise amount of diluent. Inject the diluent slowly down the inner wall of the vial to prevent foaming, which can denature protein-based drugs or trap medication in the bubbles.
4. Gentle MixingGently roll the vial between your palms until the powder is completely dissolved and the solution is clear. Never shake vigorously, as this can cause mechanical damage to the drug molecules and create excessive bubbles that make accurate dose withdrawal difficult.
5. Final Inspection and LabelingInspect the reconstituted solution for particulate matter, cloudiness, or discoloration. Label the vial with the date, time of reconstitution, new concentration, and your initials. Note the beyond-use date based on pharmacy policy, as reconstituted drugs often have limited stability at room temperature or under refrigeration.
Safety Alert: Always treat a medication vial as single-dose unless it is specifically labeled as multi-dose. Never pool leftover contents from single-dose vials for later use on the same or another patient.
