Auscultate bilateral breath sounds while simultaneously observing chest wall movement to immediately differentiate between a displaced endotracheal tube, a tension pneumothorax, or a circuit obstruction.
A tension pneumothorax presents with absent breath sounds and absent chest rise on the affected side, often with tracheal deviation. This requires immediate needle decompression. Right mainstem intubation presents with absent or diminished sounds on the left but visible chest rise on the right; the tube should be withdrawn slightly.
Do not delay assessment to check the ventilator circuit first. If breath sounds are bilaterally absent and there is no chest rise, immediately disconnect the patient from the ventilator and begin manual bag-valve-mask ventilation to rule out a complete circuit obstruction versus a patient emergency.
