In a verbal child, dysuria, frequency, and urgency are classic localizing symptoms. For preverbal children, you must rely on objective urine findings and a high index of suspicion when parents report crying during urination.
Fever may be the only sign in children under 2 years. Do not rule out UTI based on the absence of urinary complaints. A bagged specimen is only useful for ruling out infection; a positive culture requires confirmation by catheterization or suprapubic aspiration.
