Perform neurological checks every 15-30 minutes, monitoring for Cushing's triad (bradycardia, hypertension, irregular respirations) and pupillary changes. Keep the head midline and elevate the bed to 30 degrees to promote venous drainage.
Do not cluster nursing activities or allow straining (coughing, vomiting, Valsalva) as these transiently spike ICP. Implement seizure precautions with padded side rails and emergency airway equipment at the bedside.
