Assessment of lymphadenopathy in pediatric Hodgkin's disease
해설
Painless lymphadenopathy, particularly in the mediastinal and cervical regions, is the most characteristic presenting sign of Hodgkin's disease in children.

Hodgkin's disease (Hodgkin lymphoma) is a malignant tumor of the lymphatic system that commonly occurs in children and adolescents. The most characteristic and frequently observed assessment finding in pediatric Hodgkin's disease is painless lymph node enlargement, with involvement of the mediastinal (chest) and cervical (neck) lymph nodes being particularly characteristic.

The pathophysiological mechanism of Hodgkin's disease involves the abnormal proliferation of Reed-Sternberg cells within the lymph nodes, causing the affected lymph nodes to progressively enlarge. Unlike infectious lymphadenopathy, these enlarged lymph nodes are generally painless, firm, and have a rubbery texture. The disease often spreads in an orderly fashion from one lymph node group to adjacent groups, making the pattern of lymph node involvement diagnostically significant.

In pediatric patients, mediastinal lymph nodes are frequently involved, which can be detected through chest X-rays or CT scans. Cervical and supraclavicular lymph nodes are also commonly involved and can be palpated during physical examination. The painless nature of these enlarged lymph nodes is important for distinguishing them from infectious causes, which typically cause tender lymphadenopathy.

Although some pediatric patients with Hodgkin's disease may present with systemic B symptoms (fever, night sweats, weight loss), painless lymph node enlargement remains the most consistent and characteristic finding. Early recognition of this key assessment finding is essential for prompt diagnosis and initiation of treatment, which significantly improves the prognosis of pediatric Hodgkin's disease.
