MOE is an invasive infection that can rapidly erode the temporal bone, causing skull base osteomyelitis. The danger lies in its potential for locoregional spread, leading to cranial nerve palsies (most commonly the facial nerve) and vascular complications.
A normal tympanic membrane (pearly gray) or benign findings like cerumen impaction do not rule out a deeper, invasive process if purulent drainage is present. Do not delay reporting to perform irrigation or other interventions that could worsen the infection.
