The sudden absence of thrill and bruit is the cardinal sign of acute thrombosis or severe stenosis. This is a vascular access emergency requiring immediate reporting and intervention to restore flow within hours to prevent permanent access loss.
Do not confuse normal postoperative findings with an emergency. Slight swelling or warmth is expected. A palpable pulse is normal, but a thrill must be continuous. Never take a blood pressure or perform venipuncture on the access arm.
