Assess for the 6 Ps: Pain, Pallor, Paresthesia, Poikilothermia, Paralysis, and Pulselessness. Pulselessness and paralysis are late signs indicating irreversible tissue necrosis. A palpable pulse does not rule out ACS.
Normal compartment pressure is 0-8 mmHg. Pressures exceeding 30 mmHg or within 30 mmHg of diastolic blood pressure require emergency fasciotomy to restore perfusion.
Never elevate the limb above heart level or apply ice; elevation reduces arterial inflow and ice causes vasoconstriction, worsening ischemia. Keep the limb at heart level and immediately notify the provider.
