Post-Thyroidectomy Complication Monitoring
Following a total thyroidectomy, one of the most critical nursing assessments is monitoring for signs of
hypocalcemia secondary to inadvertent removal or damage to the
parathyroid glands. These small glands, located on the posterior aspect of the thyroid, regulate serum calcium levels via parathyroid hormone (PTH). Surgical trauma, devascularization, or accidental excision leads to a rapid decline in PTH, causing an acute drop in serum calcium within the first 24 to 48 hours postoperatively [1, 3].
The priority assessment finding indicating a potential complication is a
positive Chvostek's sign and perioral numbness. These are classic clinical manifestations of
neuromuscular irritability caused by hypocalcemia. When serum calcium levels fall, the threshold for neuronal sodium permeability decreases, leading to spontaneous and repetitive depolarization of sensory and motor nerves. This pathophysiological mechanism manifests as perioral paresthesia, tingling in the fingertips, and a positive Chvostek's sign (twitching of the ipsilateral facial muscles upon tapping the facial nerve anterior to the ear). If uncorrected, this hyperexcitability can progress to carpopedal spasm (
Trousseau's sign), laryngospasm, and generalized seizures [1, 2]. The synergistic effect of postoperative hypocalcemia can precipitate severe neurological events, including convulsive episodes and acute psychosis, as metabolic derangements directly impact central nervous system function
[2]. In rare cases, chronic hypoparathyroidism can lead to calcification of the basal ganglia (
Fahr syndrome), resulting in a spectrum of neurological manifestations
[4].
The other findings, while requiring nursing attention, do not represent the same level of immediate physiological threat. Hoarseness and difficulty speaking are common and expected after thyroidectomy due to endotracheal tube irritation or recurrent laryngeal nerve manipulation, but a positive Chvostek's sign signals a life-threatening electrolyte imbalance. Mild incisional pain rated 4/10 is an anticipated postoperative finding managed with analgesics. A low-grade temperature of
100.2°F (37.9°C) in the first 12 hours may indicate atelectasis or a non-infectious inflammatory response and is not the priority over impending tetany. Immediate recognition of hypocalcemia signs allows for prompt intervention with intravenous calcium gluconate to prevent airway compromise and cardiac dysrhythmias, which is why this assessment takes precedence in the clinical setting [1, 3].
