Understanding the Mechanism
Warfarin is a
vitamin K antagonist (VKA). It works by inhibiting the enzyme vitamin K epoxide reductase, which is essential for the gamma-carboxylation of vitamin K-dependent clotting factors. This process blocks the activation of Factors
II (Prothrombin),
VII,
IX, and
X, thereby reducing the functional levels of these factors in the circulation and lowering the blood's potential to clot
[1]. When a patient experiences severe bleeding from a warfarin overdose, the administration of exogenous vitamin K bypasses this blockade, allowing the liver to synthesize new, functional clotting factors.
Why Prothrombin Time is the Correct Answer
The effectiveness of vitamin K in reversing warfarin toxicity is best evaluated by monitoring the
Prothrombin Time (PT), typically reported as the
International Normalized Ratio (INR). The PT/INR is specifically sensitive to deficiencies in three of the four vitamin K-dependent clotting factors: Factor II, Factor VII, and Factor X. Because Factor VII has the shortest half-life of these factors, the PT is the first and most direct laboratory indicator to reflect the restoration of clotting factor synthesis following vitamin K administration. VKA therapy requires careful monitoring because pharmacological responses vary significantly between individuals, and the PT/INR is the standard test used to guide this therapy and assess its reversal

Analyzing the Other Options

Hemoglobin and hematocrit levels (Option 1) are indicators of oxygen-carrying capacity and red blood cell mass, not the functional status of the coagulation cascade. While they are crucial for assessing the severity of ongoing blood loss, they will not immediately reflect the pharmacological reversal of warfarin's anticoagulant effect.

Platelet count and bleeding time (Option 2) evaluate primary hemostasis, which involves platelet plug formation.
