The LPN can reinforce previously taught information, such as pursed-lip breathing for a client with COPD who is being discharged. The client with acute confusion and hypoglycemia is unstable and requires initial assessment by the RN. Initial ambulation after surgery involves evaluation of a new activity, which is the RN's responsibility. The client with heart failure and new crackles with low oxygen saturation is unstable and requires immediate RN assessment and intervention.
