Pneumonia / 폐렴

Pneumonia is infection/inflammation of the lung parenchyma. Common signs include fever, productive cough, pleuritic chest pain, crackles on auscultation, increased respiratory rate, and hypoxemia. The PT role centers on airway clearance when there is confirmed secretion retention or impaired clearance, positioning to optimize ventilation, and early, monitored mobilization as the patient tolerates. Airway clearance is not automatic for every pneumonia — it is indicated when secretions are genuinely present and the patient cannot clear them effectively.

폐렴(pneumonia)은 폐실질의 감염·염증입니다. 흔한 징후로 발열, 가래 동반 기침, 흉막성 흉통, 청진 시 수포음(crackles), 호흡수 증가, 저산소혈증이 있습니다. 물리치료의 역할은 분비물 저류가 확인되었거나 청결 능력이 저하되었을 때의 기도청결(airway clearance), 환기를 최적화하는 체위, 그리고 환자가 견디는 범위에서의 조기·모니터링 하 조기 이동에 있습니다. 기도청결은 모든 폐렴에 자동으로 적용되는 것이 아니라, 실제로 분비물이 있고 환자가 스스로 효과적으로 배출하지 못할 때 적응증이 됩니다.

Restrictive diseases / 제한성 질환

Restrictive patterns arise from several mechanisms: parenchymal (e.g., pulmonary fibrosis — stiff, scarred lung with reduced compliance and diffusion), chest-wall restriction (e.g., kyphoscoliosis, obesity, post-surgical splinting), and neuromuscular weakness (e.g., weak respiratory muscles reducing the ability to expand the lungs and cough). Presentation typically includes dyspnea on exertion, rapid shallow breathing, and reduced exercise tolerance.

제한성 패턴은 여러 기전에서 비롯됩니다: 실질성(예: 폐섬유증(pulmonary fibrosis) — 순응도와 확산이 감소한 뻣뻣하고 흉터진 폐), 흉벽(chest-wall) 제한(예: 척추후측만증, 비만, 수술 후 통증성 부목 자세), 그리고 신경근(neuromuscular) 약화(예: 호흡근 약화로 폐 확장과 기침 능력 저하)입니다. 임상 양상은 대체로 노작성 호흡곤란, 빠르고 얕은 호흡, 운동내성 저하를 포함합니다.
