NCLEX PART 3

CH 08. Maternal & Newborn
(OB & Labor)

In OB, you have two patients: Mom and Baby.

The NCLEX focuses on Safety during labor.
If the baby's heart rate drops, you need to know WHY and WHAT to do.
Memorize VEAL CHOP and the LION intervention!

1. Prenatal Calculation (Math)

You may get a calculation question about the due date or history.

  • Naegele’s Rule (Due Date):
    First Day of LMP + 7 Days - 3 Months + 1 Year
    (Example: LMP June 10 -> June 17 -> March 17)
  • GTPAL System:
    - Gravida (Total pregnancies, including current)
    - Term (Born at 37+ weeks)
    - Preterm (Born 20-37 weeks)
    - Abortions/Miscarriages (< 20 weeks)
    - Living children

2. Fetal Heart Rate Patterns

Look at the monitor. Is the baby stressed? (Normal FHR: 110-160 bpm)

Pattern Cause Nursing Intervention
Variable Decelerations
(V shape, abrupt)		 Cord Compression
(Baby is squeezing the cord)		 Reposition Mom
(Side-lying or Knee-chest).
Give O2.
Early Decelerations
(Mirror contraction)		 Head Compression
(Baby is moving down)		 None (Document)
This is normal as labor progresses. Prepare for delivery.
Accelerations
(HR goes up)		 Okay (Oxygenated)
(Happy baby)		 None
This is a good sign!
Late Decelerations
(Starts after contraction peak)		 Placental Insufficiency
(Baby is NOT getting O2!)		 EMERGENCY!
Execute LION protocol immediately.

3. Interventions for Late Decels

When you see Late Decelerations, do L.I.O.N. immediately!

  • L - Lie on Side (Left side preferred) to fix blood flow.
    I - Increase IV fluids (fix hypotension).
    O - Oxygen (8-10L via mask).
    N - Notify Provider.
    *(Stop Pitocin if running!)*

4. Postpartum & Newborn Care

① Postpartum Hemorrhage

  • Boggy Uterus:
    If the fundus is soft (boggy), MASSAGE it first!
    If displaced to the side, the bladder is full -> Have patient void.

② Newborn (APGAR)

Checked at 1 min and 5 min. (0-2 points each, Max 10)

  • Appearance (Color: Pink = 2, Blue limbs = 1)
    Pulse (>100 = 2)
    Grimace (Cries/Sneeze = 2)
    Activity (Active motion = 2)
    Respiration (Strong cry = 2)

VEAL CHOP is your best friend in the exam.
Remember: Late Decels = BAD = LION.
