NCLEX Review Guide: Pancreatic Enzyme Replacements

Overview and Indications

• Pancreatic enzyme replacements are medications that supplement or replace the natural digestive enzymes (lipase, protease, amylase) produced by the pancreas when pancreatic function is impaired.
• Primary indications include chronic pancreatitis, cystic fibrosis, pancreatectomy, and pancreatic cancer where exocrine pancreatic insufficiency occurs.
• These enzymes are essential for proper digestion and absorption of fats, proteins, and carbohydrates in the small intestine.

Primary Agents

• Pancrelipase (Creon, Pancreaze, Zenpep) is the most commonly prescribed pancreatic enzyme replacement containing lipase, protease, and amylase.
• Capsules contain enteric-coated microspheres that protect enzymes from gastric acid and release them in the alkaline environment of the duodenum.
• Never crush or chew capsules - this destroys the enteric coating and inactivates enzymes in stomach acid.

1. Administer immediately before or during meals and snacks
2. Swallow capsules whole with adequate fluid
3. For patients unable to swallow: open capsule and sprinkle contents on soft, acidic food (applesauce, yogurt)
4. Do not mix with alkaline foods or liquids that may inactivate enzymes

Assessment and Monitoring

• Monitor for steatorrhea (fatty, foul-smelling stools), weight loss, and malnutrition as indicators of inadequate enzyme replacement.
• Assess nutritional status including fat-soluble vitamin levels (A, D, E, K) as malabsorption can lead to deficiencies.
• High doses may cause fibrosing colonopathy, particularly in cystic fibrosis patients - monitor for abdominal pain and constipation.

Essential Teaching Points

• Educate patients to take enzymes with every meal and snack containing fat - even small amounts require enzyme supplementation.
• Teach proper storage in cool, dry place and to check expiration dates as enzymes lose potency over time.
• Instruct patients to report persistent steatorrhea, abdominal pain, or weight loss as these may indicate need for dose adjustment.
