ABCs (Airway, Breathing, Circulation)

NCLEX Review Guide: Leadership Management, Prioritization & ABCs

Leadership & Management Principles

Core Leadership Responsibilities

  • Delegation involves transferring responsibility while maintaining accountability for patient outcomes and safety.
  • Supervision requires ongoing oversight of unlicensed personnel and ensuring competent task completion.
  • Conflict Resolution uses therapeutic communication and collaborative problem-solving techniques.

Key Points

  • RNs can delegate tasks but cannot delegate nursing judgment or assessment
  • Five Rights of Delegation: Right task, person, circumstances, direction, supervision

Prioritization & Clinical Judgment

Priority Setting Frameworks

  • Maslow's Hierarchy: Physiological needs first, then safety, followed by psychosocial needs.
  • ABC Priority: Airway takes precedence over breathing, which takes precedence over circulation.
  • Acute vs. Chronic: Always prioritize acute, life-threatening conditions over chronic stable conditions.

Memory Aid: CURE Priority

Critical/Life-threatening → Urgent → Routine → Elective

High vs. Low Priority Conditions

High PriorityLow Priority
Airway obstructionChronic pain
Respiratory distressConstipation
HemorrhageAnxiety
Chest painInsomnia

ABCs: Airway, Breathing, Circulation

Airway Assessment & Management

  • Airway patency is the absolute first priority in any emergency or assessment situation.
  • Signs of airway compromise: Stridor, inability to speak, drooling, tripod positioning.
  1. Look for visible obstruction or swelling
  2. Listen for abnormal breath sounds (stridor, wheezing)
  3. Assess ability to speak clearly
  4. Position patient appropriately (head-tilt, chin-lift if no spinal injury)

Breathing Assessment

  • Respiratory rate, depth, and rhythm should be assessed after ensuring airway patency.
  • Critical signs: Rate <12 or >20, use of accessory muscles, cyanosis, decreased oxygen saturation.

Circulation Assessment

  • Pulse, blood pressure, and perfusion are assessed after airway and breathing are secured.
  • Shock indicators: Weak pulse, hypotension, cool/clammy skin, altered mental status.

Clinical Scenario

You enter a patient's room and find them unconscious. Using ABC priority:

  1. Airway: Check for obstruction, position head appropriately
  2. Breathing: Assess respiratory effort and provide ventilation if needed
  3. Circulation: Check pulse, begin CPR if no pulse detected

Commonly Confused Concepts

Delegation vs. Assignment

DelegationAssignment
Transferring task to unlicensed personnelDistributing work among licensed staff
RN retains accountabilityEach person accountable for own work
Requires supervisionPeer-level responsibility

ABC Memory Aid

Always check Before Continuing - Airway Before Breathing Before Circulation

Study Tips & Quick Checks

Priority Questions Strategy

  • Identify the most life-threatening option first
  • Use ABC framework when multiple patients need attention
  • Consider stability vs. instability - unstable patients always come first

Leadership Memory Aid: LEAD

Listen actively → Evaluate options → Act decisively → Delegate appropriately

Quick Check: Priority Scenario

Four patients need attention: (A) Chest pain, (B) Chronic back pain, (C) Difficulty breathing, (D) Anxiety about surgery

Correct order: C (breathing), A (circulation), D (safety), B (comfort)

Common Pitfalls to Avoid

  • Don't prioritize chronic conditions over acute ones
  • Never delegate assessment, teaching, or unstable patients
  • Always consider ABC before psychosocial needs

Self-Assessment Checklist

  • ☐ Can I identify the five rights of delegation?
  • ☐ Do I understand ABC priority framework?
  • ☐ Can I differentiate between acute and chronic priority levels?
  • ☐ Do I know signs of airway, breathing, and circulation compromise?

Remember: You've got this! Trust your nursing judgment, use systematic approaches like ABC, and prioritize patient safety above all. Your dedication to learning these concepts will make you an excellent nurse leader!
