Contact Dermatitis Mechanism

• Urushiol is the allergenic oil found in poison ivy, oak, and sumac that causes type IV delayed hypersensitivity reactions. This oil can remain active on clothing, tools, and pet fur for months to years.
• Symptoms typically appear 12-48 hours after exposure and include intense pruritus, erythema, vesicles, and possible bullae formation in severe cases.

Immediate Care

1. Remove contaminated clothing and wash with hot, soapy water immediately
2. Rinse affected skin with cool water within 10 minutes of exposure if possible
3. Apply cool, wet compresses for 15-20 minutes several times daily
4. Administer prescribed topical corticosteroids for localized reactions

Pharmacological Interventions

• Topical corticosteroids (hydrocortisone 1-2.5%) are first-line treatment for mild to moderate reactions, applied 2-3 times daily for 7-10 days maximum.
• Oral prednisone may be prescribed for severe, widespread reactions (>10% body surface area) or facial involvement, typically 1mg/kg/day for 10-14 days with gradual taper.
• Oral antihistamines like diphenhydramine or loratadine help control pruritus but don't treat the underlying inflammation.

Prevention Strategies

• Teach patients to recognize the "leaves of three, let it be" rule, though poison sumac has 7-13 leaflets per stem.
• Recommend wearing long sleeves, pants, and gloves when hiking or gardening in areas where these plants may grow.
• Emphasize washing all clothing, tools, and pets that may have contacted the plants using hot, soapy water.
